This page allows you to search for participant eligibility status with the following search criteria:
SSN – Social Security Number of the participant.
Member ID – Social Security Number of the participant (unless a unique nine digit ID appears on the participant's ID Card).
When complete, select
The participant and a list of his/her dependents will be displayed. Select the participant or dependent for whom you wish to view eligibility. A list of coverage and eligibility will be displayed.
