Pension Fund – Name of the Pension Fund.
Document Type – Name of the document.
Message – Message for this selected pension fund and document type.
Choose File – Select a file to upload, accepted file formats are .pdf .tif .jpeg .jpg .png and .bmp. To upload more document(s) for same or another pension fund, repeat the above steps.
When complete, select
If you are successful in submitting your document(s), you will see the following message:
Thank you for uploading the documents. To upload more documents click here.
You will receive an email acknowledging the submission of these documents.
