This page allows you to search for participant claims with the following search criteria:
Patient Account Number – Account number assigned to the patient by the Provider.
SSN – Social Security Number of the participant.
Member ID – Social Security Number of the participant (unless a unique nine digit ID appears on the participant's ID Card).
Check No – Check number issued for the participant’s claims.
Patient First Name – First name of the patient.
Patient Last Name – Last name of the patient.
Patient Date of Birth – Date of birth of the patient.
When complete, select
The claim(s) matching the criteria input (Patient Account Number, SSN, Member ID, Check No, Patient First Name, Patient Last Name, or Patient Date of Birth) will be displayed.
To view an Explanation of Benefits (EOB) for a particular claim, select View Details.
If there are any questions or comments on claim(s), please select the claim(s) and select Inquire.
You cannot select more than five (5) claims to inquire.
